Reablement Worker

Reablement Service

£13.09 per hour / £24,581 pro rata

Hours: 30 Per Week

Golden Hello: £1500 (£500 upon start, £1000 upon completion of probational period)

We are looking for caring and self-motivated individuals who have a passion for improving people’s lives to join our highly reputable Reablement service. This service has been running in Sefton for a number of years and is highly reputable within the Sefton community; we support and work in partnership with the NHS and Sefton Council directly, to enable people to live independently at home for longer.

Being a Reablement Health Care Worker is so much more rewarding than you think. In this role, you aren’t taking care of people in a healthcare setting – you are helping them to manage their own personal care within their own homes. By focussing on abilities and outcomes, you are assisting them to improve their long-term independence.

Support Worker

The North Hub is a large day service that provides quality care and support for adults who have learning, physical disabilities, autism or associated mental health conditions. The Hub has another satellite site within the locality where service users participate in horticulture activities.

Support Worker

Poplars, Southport

An exciting opportunity has arisen to join our Maintenance team. The post holder is required to carry out interior and exterior building maintenance to a high standard to meet statutory requirements. The post holder will also be involved in a programme of general daily maintenance, cleaning, repair work and decoration to the interior and exterior of various New Directions sites in the South of the borough, ensuring that the security of buildings is maintained in accordance with operational requirements.

Experience of carrying out simple repair and maintenance work is essential. Health and Safety training will be provided.

Maintenance Operative

Location: Supported Living Houses
4 Sefton Locations

Based in Southport, our North Hub is a day activity hub for adults who have additional support needs due to learning and/or physical disabilities, autistic spectrum disorder and people who require age related support.

The successful candidate will provide a programme of personal and social care, according to a Care Plan, specific to each individual service user. Care duties will include personal care, dietary care and a range of community based daily living tasks and therapeutic activities. You will also work with service users, carrying out all aspects of their Care Plan to achieve agreed outcomes.

Contract Administrator

Chase Heys in Southport offers bed-based assessment and Intermediate Care Service supporting discharges from Southport and Formby District General Hospital.

The successful candidate will provide a range of administrative and IT functions to support the

operations at Chase Heys. You will be responsible for the collating and monitoring of data in line with occupancy and discharge. You will be required to manage incoming calls, ensuring these are effectively communicated to the relevant parties and also to provide an efficient reception service.
