TRUSTED ASSESSOR – (12-month fixed term contract)

Sefton’s leading care provider, New Directions, is now recruiting a Trusted Assessor based in Netherton. This is an exciting time to join a new team that will enhance the well-established Reablement service.
The post holder will contribute to the day-to-day management of the Crisis Support service by working as an integrated member of the multi-disciplinary team in the community, usually in service users’ homes. Promote independence and choice for service users either by directly helping them to work towards their outcome-focused goals as set out in their support plan or through the coordination and supervision of a team of Crisis Support Workers to deliver a high-quality service.
Using person-centred approaches work closely with service users, carers and other professionals to meet the individuals’ needs, improving discharge and transfer from the hospital and helping to reduce avoidable admissions/ readmissions to hospital or residential care.
The post holder is required to participate in a 5/7 rota to cover the duty phone during evenings and weekends. The post holder will also provide support to the Reablement and Domiciliary Care teams during busy periods. Must be able to fulfil the travel requirements of the post.
