- Seamless Transitions Home: Our Chase Heys Intermediate Care and James Dixon Court Enabling Unit provide a smooth transition from hospital to home, easing the burden on the NHS and ensuring a comfortable recovery for patients.
- Independent Living: Our Shared Lives Scheme allows people with disabilities to flourish in inclusive, Family-Based Sefton communities. Thanks to the incredible support they receive, many participants have secured jobs and volunteered.
