Reablement Worker

Reablement Service

£13.09 per hour / £24,581 pro rata

Hours: 30 Per Week

Golden Hello: £1500 (£500 upon start, £1000 upon completion of probational period)

We are looking for caring and self-motivated individuals who have a passion for improving people’s lives to join our highly reputable Reablement service. This service has been running in Sefton for a number of years and is highly reputable within the Sefton community; we support and work in partnership with the NHS and Sefton Council directly, to enable people to live independently at home for longer.

Being a Reablement Health Care Worker is so much more rewarding than you think. In this role, you aren’t taking care of people in a healthcare setting – you are helping them to manage their own personal care within their own homes. By focussing on abilities and outcomes, you are assisting them to improve their long-term independence.

Team Leader

Chase Heys, Southport

The post holder will work as part of an integrated team delivering care and support in a number of different settings. You will contribute to meeting the assessed care needs of service users in a way that respects the dignity of the individual and promotes independence, demonstrating a sensitive, understanding and person centred approach.

The post holder will assist the manager in the day to day running of the service, ensuring an environment is provided where service users feel secure and comfortable. You will be responsible for supervising the daily work routine of care staff, ensuring that they are meeting the requirements of the post.

Support Worker

North Hub, Southport

The North Hub is a large day service that provides quality care and support for adults who have learning, physical disabilities, autism or associated mental health conditions. The Hub has another satellite site within the locality where service users participate in horticulture activities.

The successful candidate will participate in the day-to-day delivery of a range of services for people with learning disabilities, physical disabilities, autism or associated mental health conditions; working with service users in a person-centred way to promote greater choice, skill development, independence and inclusion. The post holder will work in a variety of community-based settings, including day service activities.

An exciting opportunity has arisen to join our team at Chase Heys. Working as part of an integrated team delivering care and housing-related support in a Respite and Intermediate Care home. Contribute to meeting the assessed care needs of service users in a way that respects the dignity of the individual and promotes independence, demonstrating a sensitive, understanding and patient approach.

The successful candidate will provide a programme of personal and social care, according to a Care Plan, specific to each individual service user. Care duties will include personal care, dietary care and a range of community-based daily living tasks and therapeutic activities. You will also work with service users, carrying out all aspects of their Care Plan to achieve agreed outcomes.
