What is Reablement?
Our Reablement service is built on the Trusted Assessor model. Our team provides comprehensive support and personalised care plans focused on “recovery through enablement. Trusted Assessors design these plans and source and deliver the equipment and medication technology you may need to achieve reablement and live independently.
The beauty of Reablement is that many people we support require no ongoing care or experience a significant reduction in their long-term needs. This empowers individuals and eases pressure in the broader health and social care system.
Crisis Support: Averting Crisis and Promoting Stability
New Directions also offers Crisis Support, a service that extends a helping hand to community members facing a crisis. This includes individuals we already support and their carers, friends, and family. Our Crisis Support team intervenes with short-term solutions that prevent the need for long-term social care or hospital admissions.
Specifically, our Crisis Support service focuses on providing Domiciliary Care, Reablement tasks, and practical support. This allows people who are medically well to remain safe and secure in their homes.
Spreading the Word: A Multi-Channel Approach
To raise awareness of the Reablement Team and its services, we’ve launched a comprehensive marketing campaign that includes:
- Bus Ads: Reaching residents across North Liverpool and Southport.
- Radio & Spotify Audio Ads: Reaching a wider audience through familiar platforms.
- Flyers: Distributed to 26,000 residential addresses in Liverpool for targeted outreach.
- Video Testimonials: Featuring members of the Reablement Team discussing their work, produced in collaboration with Sefton Council Communications and shared on social media.

Here’s a glimpse into what you’ll be doing:
- Supporting Discharge and Reducing Readmissions: You’ll play a key role in helping people transition smoothly from the hospital back to their homes, reducing the risk of readmission.
- Promoting Independence: As part of a multidisciplinary team, you’ll help individuals regain or maintain daily living skills, including personal hygiene, meal preparation, and medication management. You’ll also encourage social and leisure activities to promote overall well-being.
- Building Relationships and Risk Management: It is key to develop trusting relationships with those you support. You’ll also work to identify and manage potential risks to ensure their safety and well-being.
- Supporting Others: You’ll be a role model for new team members, sharing your knowledge and experience. You’ll also assist Trusted Assessors with their tasks.
